Healthcare Decision Making: Learn the Basics and Complete Your Forms

How can you communicate your medical wishes if you're unable to make decisions for yourself?  Learn about the Durable Power of Attorney for Healthcare and Living Will to help your loved ones and healthcare providers know your treatment preferences.

Bring your questions and we'll provide booklets to complete for your advanced care planning.  You can take the booklet home, or you can put your preferences in writing that day.  There will be notaries on site for you to sign your forms.  You will receive the originals and an extra copy to share with your loved ones and healthcare providers.  If you are a patient of Southern NH Health, we will upload your information to MyChart on your behalf.  The program runs from 11am-1pm.  Pre-registration is helpful.  Call the Front Desk at 603-889-6155 and sign up for either the 11am-12pm block or 12pm-1pm block.
